COVID VACCINE

Salud is only offering the COVID vaccine to established Salud patients.

COVID VACCINE FOR CHILDREN 5 YEARS OF AGE AND OLDER:

Salud is excited to offer the vaccine to children 5+ years of age!

The CDC now recommends that children between the ages of 5 and 11 years receive the Pfizer-BioNTech pediatric COVID-19 Vaccine. You can find more information and read CDC’s media statement.

CONSENT FORM:

A minor consent form is needed to administer the Pfizer vaccine to any child ages 5+.

If you are the parent/ guardian and accompanying your child to his/ her vaccine appointment, you can fill out and sign at the clinic.

If you are NOT going to be present for your child’s vaccination, please print out and fill out the minor consent form.

Salud needs this filled out and signed by the parent and/ or guardian to administer the vaccine.

COVID "BOOSTER" SHOTS FOR ANYONE 18+ YEARS OF AGE:

  • Salud is providing the Pfizer and Moderna "booster" shot to any Salud patient who is 18 years of age or older.

  • Given Colorado’s high rate of COVID-19 transmission, Governor Polis issued Executive Order 2021-137  declaring the entire state of Colorado high risk for exposure or transmission of COVID-19 for the purpose of COVID-19 vaccine booster eligibility, encouraging all Coloradans who are 18+ to get a COVID-19 vaccine booster dose six months after receiving their initial series of Pfizer or Moderna vaccine or two months after receiving the J&J vaccine. 

  • CDPHE’s booster guidance for individuals is on their website.

ADDITIONAL DOSE FOR IMMUNOCOMPROMISED PEOPLE:

  • If you are immunocompromised, a third dose is recommended.

  • Definition of Immunocompromised People:​

    • Active treatment for solid tumor and hematologic malignancies.

    • Receipt of solid-organ transplant and taking immunosuppressive therapy.

    • Receipt of CAR-T-cell or hematopoietic stem cell transplant (within 2 years of transplantation
      or taking immunosuppression therapy).

    • Moderate or severe primary immunodeficiency (e.g., DiGeorge syndrome, Wiskott-Aldrich
      syndrome).

    • Advanced or untreated HIV infection.

    • Active treatment with high dose corticosteroids (i.e., ≥20mg prednisone or equivalent per day),
      alkylating agents, antimetabolites, transplant-related immunosuppressive drugs, cancer
      chemotherapeutic agents classified as severely immunosuppressive, tumor-necrosis (TNF)
      blockers, and other biologic agents that are immunosuppressive or immunomodulatory.​

PLEASE BRING THE FOLLOWING TO YOUR SCHEDULED VACCINE APPOINTMENT:

  • We require wearing your mask in all Salud clinics.

  • Insurance card, if you have one (please know, the vaccine is completely FREE for you to receive)

    • Why are you asking me to bring my insurance card in if the vaccine is free?

This can be confusing, but please know the COVID vaccine is completely FREE to you.

You do NOT need insurance to receive the vaccine.

But if you DO have insurance, Salud wants the insurance to cover the costs involved in providing the vaccine such as refrigeration of the vaccine, staff time to schedule and give the shot. Again, this will not be paid by you. Salud needs to bill the insurance to cover the costs associated with providing the vaccine.

VISITING SALUD CLINICS:

For your health and safety, we are only allowing patients with a scheduled appointment into our clinics. We are not accepting walk-ins. If you would like to schedule an appointment or have any other questions, please call 303-697-2583 or visit the PATIENT PORTAL.

Check In:

  • You must wear a mask over your nose, mouth & chin while in the clinic (children under 2 exempt).

  • Salud is continuing to restrict the number of individuals in our clinics, however we allow one additional person to attend an OB or ultrasound visit, and two caretakers to attend well child checks.

OTHER INFORMATION ABOUT THE COVID VACCINE:

  • Whether you are a U.S. citizen or not, we are all in this together. An ID is not required to receive the vaccine.​ State and local public health agencies will never share your information for any immigration or law enforcement purposes. Receiving the COVID-19 vaccine will not count against you in any public charge determinations.

  • Like other routine vaccinations, you will need to share some personal information with your provider when you get your COVID-19 vaccine. This information will only be used for public health purposes. It may include your name, date of birth, and contact information. Your information will remain protected.

  • Sharing your identity and some of your medical history ensures that the vaccine is administered safely, effectively, and responsibly. Your immunization records are confidential, personal medical information, and public health will protect it.
