Vaccine is available based on how much vaccine Salud receives each week. You must have a scheduled appointment, NO WALK-INS.
WHO IS CURRENTLY ELIGIBLE FOR THE VACCINE:
EFFECTIVE 3/1/2021: Salud is ONLY offering COVID vaccine to anyone who is 65 years of age or older and health care workers.
In order to register, you must meet this criteria. Please continue to check here for details about our vaccine eligibility.
You need to have 2 weeks between receiving any other vaccination (flu, shingles, etc.) and the COVID19 Vaccine. Please wait to register.
You do not need to be a Salud patient.
HOW TO REGISTER FOR YOUR VACCINE APPOINTMENT:

Following registration, Salud will contact you to schedule (date, time & location) your COVID vaccine appointment.
It is important that you keep your appointment. If you need to cancel, please call (303) 655-4910, so we can schedule another person.
Please do not call at this time about vaccine availability. Our Call Center is dedicated to scheduling patients for regular health visits.
PLEASE BRING THE FOLLOWING TO YOUR SCHEDULED VACCINE APPOINTMENT:
proof of occupation (to verify you are eligible to receive the vaccine)
insurance card, if you have one (please know, the vaccine is completely free for you to receive)
remember to wear your mask
COVID VACCINE EQUITY:
The state is moving as quickly as federal supplies allow. We are vaccinating groups that will have the greatest impact for all Coloradans the fastest. We will grow our list of providers giving vaccines as our supplies of vaccines grows.
Whether you are a U.S. citizen or not, we are all in this together. If you would like a vaccine, you will be able to get one according to what vaccine phase you are in. An ID should not be required for access to the vaccine. State and local public health agencies will never share your information for any immigration or law enforcement purposes. Receiving the COVID-19 vaccine will not count against you in any public charge determinations.
Like other routine vaccinations, you will need to share some personal information with your vaccine provider when you get a COVID-19 vaccine. This information will only be used for public health purposes. It may include your name, date of birth, and contact information. Your information will remain protected.
Sharing your identity and some of your medical history ensures that the vaccine is administered safely, effectively, and responsibly. Your immunization records are confidential, personal medical information, and public health will protect it.
The vaccine is completely free for you to receive.

The cost of your COVID vaccine is free, however, donations are always greatly appreciated and ensure we are able to continue proving vaccine to our patients and community members.
