On September 5, 2022, Salud became aware of suspicious activity in certain computer systems. Salud immediately launched an investigation, with the assistance of third-party computer specialists, to determine the nature and scope of the activity. Salud's investigation determined that there was unauthorized access to the affected systems on that date and that certain data may have been accessed or taken. Salud's investigation determined that some employee and patient personal and protected health information was potentially subject to unauthorized access.

Salud is notifying individuals whose information was determined to potentially have been involved in the incident and for whom Salud has sufficient contact information. Individuals who have questions about this incident or believe they may be impacted by this incident can contact the dedicated external call center at (855) 926-1137, Monday through Friday, 7am to 4:30pm MT, except holidays, or visit Salud's website for more information at https://www.saludclinic.org/.

Salud is providing impacted employees and patients with free credit monitoring and identify fraud protection services. Further, Salud is providing individuals with contact information for the three major credit reporting agencies, as well as providing advice on how to obtain free credit reports and how to place fraud alerts and security freezes on their credit files. While credit monitoring services monitor an individual's financial accounts for changes to the accounts' limits, billing addresses, and creation of new accounts, they do not monitor individual charges to determine if they are fraudulent, such as credit card accounts, checking and saving accounts. It would be prudent for impacted individuals to regularly review their financial accounts. If they notice any suspicious or unauthorized charges or withdrawals, they should contact their financial institution immediately.

Impacted individuals may also find information regarding identity theft, fraud alerts, security freezes and the steps they may take to protect their information by contacting the credit bureaus, the Federal Trade Commission or their state Attorney General. Instances of known or suspected identity theft should also be reported to law enforcement or the individual's state Attorney General.

The security of information in Salud's care is one of its highest priorities, and Salud has security measures in place to protect sensitive information in its care. Upon becoming aware of this incident, Salud immediately took steps to confirm the security of its systems. While Salud had policies and procedures in place at the time of incident regarding security of information, Salud is reviewing and enhancing those policies and procedures to further protect against similar incidents moving forward. Salud notified law enforcement and is cooperating with the investigation. Salud is also reporting to state and federal regulatory officials, as required.
