For anyone under the age of 18:

Anyone under the age of 18 receiving the vaccine MUST have a consent form filled out and signed by their parent or legal guardian. Please fill out and sign the below consent form, print and bring to your vaccine appointment.

Minors (anyone under the age of 18) must be accompanied by an adult to their vaccine appointment. If a minor does not bring a signed consent form to their vaccine appointment, the adult present must be their parent or legal guardian.

Additional details:

• When you receive your first vaccine dose, you will automatically be scheduled for your second appointment for the same time, 21 days later (please make sure you are available for that same time).

• Minors (anyone under the age of 18) must come to their vaccine appointment with an adult.

• We will ask for your insurance information. Regardless of your health insurance plan or insurance status, you will NOT be charged for the vaccine. If you don't have health insurance, you will still be able to get the vaccine at no cost to you. 

• Salud still requires you to wear your mask in our clinics.
