Salud Family Health Centers - Provider Network


Provider Network

Salud's primary health care services are provided by physicians, physician assistants, nurse practitioners, licensed psychologists, registered nurses, dentists, dental hygienists, case managers and health educators. Every physician employed by Salud is board certified or board eligible in family practice, pediatrics or internal medicine. All of Salud's dentists have faculty appointments at the University of Colorado Dental School.



Provider Credentialing


Salud is committed to continually evaluating and improving its systems and processes. Each center has its own Continuous Quality Improvement (CQI) process that addresses the needs and issues of the center. A System CQI Committee coordinates system-wide processes. The Credentialing and Privileging Committee handles credentialing and privileging activities, and reviews all requests from providers to perform health services and procedures.

Medical and dental student training opportunities

Salud provides extensive training opportunities for students through its agreements with a number of healthcare training institutions. These agreements allow for student rotations within the Salud system, including family practice residents, medical assistants, nursing, dental, physician assistants, nurse midwife, social work and nurse practitioner students. Salud also participates in multi-disciplinary training activities with the University of Colorado Health Science Center and Centennial Area Health Education Center (AHEC).


Salud Training Programs: Medical

  • Several Salud physicians have full-time faculty appointments with the University of Colorado Health Sciences Center (UCHSC).
  • All Salud clinics have been approved by the Commission on Family Medicine as approved sites for residents.
  • All Salud clinics are training sites for family practice residents, medical students, physician assistant students, and advanced nursing students.

Salud Training Programs: Dental
  • All University of Colorado Dental School (UCDS) students rotate through Salud's dental clinics.
  • Salud trains an average of four students a year from the University of Iowa School of Dentistry.
  • Salud trains students from Tufts University in Boston.
  • Continuing Medical Education (CME) training for Salud dentists and hygienists is offered through UCDS.

Primary Care Research
Salud providers are dedicated to research on issues of importance to their patients. Salud is currently participating in research studies on adolescent pregnancy, diabetes, mental health, HIV and rural health, HIV seroprevalence in farmworkers, and environmental health. An in-house committee has been established to assure that research is ethical and beneficial to the patient population and consistent with the Salud mission. Salud is a charter member of Colorado Research Network (Care Net) dedicated to focusing on health issues among underinsured and Hispanic people.

Guanajuato Colorado (Gua-Co) Project
Salud's research indicates that 42% of the migrants in its catchment areas in Colorado originate in Guanajuato, Mexico. In 1998, Salud established a working relationship and developed a research study collaboratively with the State of Guanajuato, Mexico to study migrant farmworkers who travel from Guanajuato to Colorado. By working across borders, Salud hopes this collaboration will improve the health of this shared population of patients.


Outreach Efforts

Immunization
Salud has been very proactive in childhood immunization efforts in the state of Colorado and has received awards for its immunization rates at several of its clinics from the "Together for Tots" program sponsored by the state health department. Salud staff represents the organization on the Colorado Child Immunization Coalition, and the Boulder County and Weld County Children's Immunization Coalitions.

Health Screening
Salud offers a variety of health screening services and programs to the community and to its employees. The programs include a month-long health screening promotion in February called "Heart Month," participation in the Channel 9 Health Fair program, assessments and preventative services in migrant schools, Headstart programs and public schools, and flu shot clinics.

AmeriCorps Program
Salud sponsors an Americorps program through the National Association of Community Health Centers' Community HealthCorps program. Salud is one of only two community health centers in Colorado that participate in this program. Through a variety of initiatives, AmeriCorp Members reach out to at-risk individuals in the community not currently being served by Salud. The Members also work to ensure follow-up on treatment plans and development of preventive health practices for new and existing Salud patients.
