Pharmacy services are available to any registered patient of Salud Family Health Centers.

The main Pharmacy is located in Fort Lupton; Salud provides pharmacy delivery to your home clinic for pick-up.

REFILLS

Please contact the Pharmacy directly for refills. We recommend calling 5 days in advance to assure that your refill is ready when you need it.

Please call the pharmacy or the pharmacy will call you to confirm pickup and payment of medication. It is important that all medications are picked up within 5 days of arrival at your home clinic. Failure to pickup medication may result in termination of delivery privileges or pre-payment prior to delivery.

We are accept Medicaid, Medicare Part D and most third party insurance.

PAYMENT OPTIONS

The Salud Pharmacy accepts Cash and Credit Cards (MC, Visa, Discover). Checks are not accepted at this time.
